Community Support Services for Children with Disabilities in West Dunbartonshire

West Dunbartonshire Health and Social Care Partnership (HSCP) is seeking an experienced and creative service provider to develop community-based support for children and young people with disabilities and their families. This opportunity is part of their 5-year strategic plan to enhance children's services. The successful provider will deliver a flexible, needs-led service that supports children at home and in their communities, using an evidence-based, whole-family approach. Services should help young people achieve their personal outcomes through individualized support and engaging activities. The HSCP is committed to incorporating family feedback and experiences into service design.
