    Brief

    Icanho delivers specialist community neurorehabilitation services for adults aged 18+ registered with GP practices in Suffolk and the Great Yarmouth and Waveney locality. The service supports individuals with acquired neurological conditions including stroke, traumatic brain injury, hypoxic brain injury, brain tumours, and other acquired brain injuries. A multidisciplinary team provides physiotherapy, occupational therapy, speech and language therapy, psychology, and vocational rehabilitation through personalised, goal-orientated interventions. The service addresses physical, cognitive, communication, behavioural, psychological, and vocational impacts of neurological injury while supporting families and carers. Icanho works collaboratively within the Norfolk and Suffolk rehabilitation pathway, partnering with acute hospitals, community health services, primary care, mental health services, social care, and voluntary organisations to enable timely access to specialist rehabilitation, facilitate safe hospital discharge, support community reintegration, and reduce unnecessary demand on health and care services through evidence-based interventions.
