Social Prescribing Link Worker Service - Health Connections

Somerset Integrated Care Board is seeking to continue contracted services for social prescribing link workers through Health Connections. Social prescribing connects people to non-clinical community activities and services that support their social, emotional, and practical wellbeing. This service is delivered by trained professionals using a holistic, person-centred approach that empowers individuals, reduces reliance on clinical services, and strengthens local communities. The contract will be awarded to the existing provider following a Provider Selection Regime Direct Award Process C, as the current provider continues to satisfy the service requirements. The service specification remains largely unchanged from the original contract. Contract period: 12 months (1 July 2026 - 30 June 2027). This opportunity is managed by NHS South, Central and West Commissioning Support Unit on behalf of Somerset Integrated Care Board.
