Home Care Reablement Workers for Lambeth Council

Lambeth Council is seeking a partner to supply reablement home care workers for its integrated reablement service. These workers will support people recovering from illness or hospital admission to regain independence, working alongside clinical staff from Guy's and St Thomas' NHS Foundation Trust and social workers from Lambeth Council. The service provides short-term support across the London Borough of Lambeth, with the main base at Ackerman Health Centre, Patmos Road, SW9 6AF. The contract is for 5 years with optional 2-year extension (two 1-year options). The contract is based on an hourly rate model with an indicative volume of 28,500 hours per year. The 5-year contract value is £3,400,000 excluding VAT, rising to £4,760,000 if all extension options are exercised. The successful provider must maintain a presence at the base location and participate in regular multidisciplinary team meetings.
