Supply of Toilet Paper and Paper Towels

This contract involves the supply of toilet paper and paper towels for a duration of 24 months. Suppliers are required to submit a detailed price list as part of their offer, which is included as Attachment 1A to the procurement documentation. To ensure compliance with the required specifications, suppliers must provide the following evidence with their bids: 1) Manufacturer's catalog sheets or other source materials (e.g., brochures, technical data, or instructions) confirming the required parameters. 2) Sample products (one of each item listed in the price list). Additionally, suppliers must submit the following documents by the bid submission deadline: a) A bid form using the template provided in Attachment 1 to the procurement documentation. b) A price form using the template provided in Attachment 1A to the procurement documentation. c) A declaration confirming that the supplier is not disqualified, using the template in Attachment 2 to the procurement documentation. d) The required evidence mentioned in Section VIII of the procurement documentation. e) A power of attorney (if applicable). Note: The documents listed in points XII. 1a) and b) are part of the bid and cannot be supplemented later. Failure to provide these documents will result in the bid being invalid. Bids must be submitted through the eZamawiajacy platform available at https://spskgruca.ezamawiajacy.pl/servlet/HomeServlet. Sample products must be delivered to the hospital's office or the Public Procurement Department between 7:00 AM and 3:00 PM.
