Supply of Waste and Clothing Bags for 24 Months

This contract involves the supply of waste bags and clothing bags for a duration of 24 months. Suppliers are required to submit a price form as part of their offer, which is detailed in Attachment 1A of the Terms of Reference. To confirm compliance with the required services, suppliers must provide the following evidence with their offer: 1) Manufacturer's catalog sheets and/or other source materials (e.g., brochures, technical data, or instructions for the offered products) that confirm the required specifications. 2) Sample bags (at least 3 samples of each type of bag, sealed together). If the supplier fails to submit the required documents listed in point VIII.1 items 1)-2) or if the submitted evidence is incomplete, the buyer will request their submission or completion within a specified timeframe. Suppliers must submit the following by the offer submission date: a) Offer form using the template in Attachment 1 of the Terms of Reference b) Price form using the template in Attachment 1A of the Terms of Reference c) A declaration from the supplier stating that they are not disqualified - according to the template in Attachment 2 of the Terms of Reference d) The required evidence mentioned in Chapter VIII of the Terms of Reference e) Power of attorney (if applicable). NOTE!!! The documents listed in point XII. 1a)b) constitute the content of the offer and cannot be supplemented. The absence of these documents renders the submitted offer invalid. Offers must be submitted via the eZamawiajacy platform available at https://spskgruca.ezamawiajacy.pl/servlet/HomeServlet. Sample bags must be delivered to the hospital's office or the Public Procurement Department (from 7:00 AM to 3:00 PM).
