    Brief

    The contract involves the supply of medical materials and implants for eye surgeries at the Międzyleski Specialist Hospital in Warsaw. The procurement is divided into 20 task groups, with quantities and assortments detailed in the Price Forms, which are integral parts of the procurement documentation. Suppliers can submit offers for all or selected tasks, but partial offers within individual tasks are not allowed. Alternative offers are also not permitted. Each supplier may submit only one offer; submitting multiple offers or alternative offers will result in the rejection of all offers from that supplier. The offered goods must comply with the description of the contract and meet the legal requirements for circulation in healthcare facilities. Failure to meet any required parameters may lead to the rejection of the offer. The contract will be executed over a period of 12 months from the date of signing a valid agreement with the selected contractor. Offers will be evaluated based on the criteria described in Chapter XVI of the procurement documentation. The draft contract provisions are included as Annex 1 to the procurement documentation. The contractor may subcontract parts of the order but must specify in the offer which parts will be subcontracted and provide the names of the subcontractors. The buyer does not require the contractor to personally perform key parts of the order. If the buyer finds grounds for exclusion regarding a subcontractor, the contractor must replace that subcontractor or withdraw the subcontracting of that part of the order. Subcontracting does not relieve the contractor of responsibility for the proper execution of the order.
