    Brief

    Hampshire is seeking a service provider to deliver community-based wellbeing support across the county through four geographical clusters. The Community Wellbeing Workers Service will provide short-term, holistic interventions to adults who would benefit from preventative or lower-level support, helping them improve financial resilience, access services, and build independence. Key service components include: financial advice and debt support; help navigating health, care, and community services; employment and volunteering support; specialized support for people with dementia and their carers; and connections to smoking cessation, substance misuse, and domestic abuse services. The service adopts a strengths-based approach, working with individuals to build on their existing skills and community assets. This is a significant opportunity for providers to deliver coordinated support across Hampshire's four clusters, with larger and more sustainable contract structures than previous arrangements. The successful provider will help reduce demand on statutory Adult Health and Care services by supporting individuals with complex needs alongside broader population health objectives.
