    Brief

The Hepatitis C Trust (HCT) is seeking to continue its established Follow Me Project, a peer-led Hepatitis C testing and treatment engagement service across Cardiff and the Vale of Glamorgan. This service targets people who use drugs and other vulnerable populations at higher risk of blood-borne viruses, particularly Hepatitis C. Key service elements include: - Peer-led outreach-based testing delivered by trained staff with lived experience - Harm reduction advice and sustained peer support - Linkage to clinical care and treatment completion support - Prevention and early diagnosis initiatives The current contract expires on 31 May 2026. This opportunity supports continuation beyond this date to maintain service continuity, improve health outcomes, reduce demand on acute services, and progress towards national Hepatitis C elimination targets. The service has demonstrated clear value and is embedded within existing clinical pathways. A direct award approach is being used due to the specialist peer-led nature of the intervention and the single capable provider.
