    Brief

    The City is seeking input from the market on a proposed Health Access Worker service to improve healthcare access for people experiencing homelessness. This is a Pre-Qualification Questionnaire (PQQ) stage engagement and does not guarantee participation in future procurement. Service Overview: The Health Access Worker will provide wraparound support connecting rough sleeping clients into the healthcare system, with lived experience as a core service component. The role is based at an Outreach provider's office but is primarily field-based, working 3 days per week with flexibility for out-of-hours client meetings. Key Objectives: • Increase GP registrations and health appointment attendance among rough sleepers • Reduce missed appointments and prevent health deterioration • Empower individuals to manage their own health and wellbeing • Maximize use of Health Emergency Bedspaces • Support NHS information-sharing pilots and inclusion health principles • Improve early identification of long-term conditions for palliative care • Amplify lived experience voices in homelessness services • Work collaboratively with partner agencies on housing stability and health outcomes Contract Details: Initial 12-month term with option to extend for 1 year. Maximum contract value: £40,000 per annum. Provider responsible for worker management and direct line supervision. Note: This procurement will comply with the Public Contracts Regulations 2015.
