Community Pre-Diabetes Prevention Service for South Bristol

NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB) is seeking a qualified provider to deliver a community-based pre-diabetes prevention programme in South Bristol. This person-centred service will support adults aged 18-50 diagnosed with pre-diabetes to prevent progression to Type 2 diabetes and improve health outcomes, with particular focus on underserved populations. Key Service Requirements: • Initial assessment and personalised support plan within 6 weeks of diagnosis • Education and lifestyle change support delivered in community settings (including evenings and weekends) • 6-month follow-up to review progress and outcomes • Navigation support linking patients to primary care, community services, and voluntary sector resources The service will complement existing programmes like the National Diabetes Prevention Programme (NDPP) and integrate with primary care. The contract offers an initial 3-year term with an optional 2-year extension, commencing 18th January 2027. Note: This procurement is managed by NHS South, Central & West (SCW) on behalf of the commissioners.
