This opportunity involves providing NHS Health Checks, a free preventive health service for eligible individuals aged 40-74 without pre-existing cardiovascular conditions who are registered with a Somerset GP or resident in Somerset. The service can be delivered to both staff and patients meeting these criteria, with a specific focus on reducing access barriers for people with severe mental illnesses by integrating health checks with mental health treatment access. As the service develops, it may extend to additional patient groups such as those accessing bariatric services. The service must follow NHS Health Checks standard protocols, including measurement of pulse rate, blood pressure, and point-of-care testing for blood cholesterol. It will identify risk factors for hypertension, diabetes, high cholesterol, kidney dysfunction, obesity, and heart arrhythmias, with findings referred to the patient's GP for consideration of formal diagnosis. The service must also provide lifestyle support and advice on alcohol consumption, smoking cessation, healthy weight management, and physical exercise.
