Home Care Support Services - Service Zone 1

The Council is conducting soft market testing for a 12-month home care services contract. Two integrated services will be delivered: (1) Home Independence Support Service (HISS) - long-term ongoing home care for people requiring support to live independently, including personal care (washing, dressing, toileting), medication support, meal assistance, mobility support, social inclusion activities, and specialized support for dementia, physical disabilities, and long-term conditions, with night-time support available; and (2) Home Independence and Crisis Support Service (HICSS) - short-term reablement and hospital discharge support including hospital-to-home transitions, admission avoidance, intermediate care, short-term reablement, active recovery post-illness or injury, crisis response, community alarm response, and assistance regaining daily living skills to reduce future care needs. Suppliers should have experience delivering person-centered care, managing complex health and social care needs, and coordinating with health services.
