Sandwell Community Dementia Support Service - Lead Provider

Sandwell is seeking a Lead Provider to deliver an integrated Community Dementia Support Service that brings together healthcare, social care, voluntary organisations, and local partners to support people living with dementia in their own communities. The Lead Provider must establish sub-contracting arrangements with local community groups and organisations that reflect Sandwell's diverse population. The service will operate as a single point of access providing: information, advice, and referrals; integrated care and support planning to promote independence; and training for people with dementia, carers, and those with memory concerns. Additionally, the service will deliver professional training to address service gaps, support the "Dementia Friendly Sandwell" initiative through community and business engagement, and work with schools and colleges to reduce stigma. The service must integrate with neighbourhood care models, primary care networks, and frailty pathways to enable early intervention, prevent crises, and help individuals maintain independence and quality of life.
