Health and Care Services Partnership Development - City and Hackney

This 12-month, time-limited funding opportunity provides £97,748.57 in S256 funds to support the City and Hackney Health and Care Services Executive Partnership Development. The initiative aims to reduce health inequalities through sustained leadership and capacity building during NHS reform. Key deliverables include: establishing an in-place-based partnership with an Innovation Academy and dedicated convening capacity; creating three transition support roles (Interim Director of Integration, Integrated Care Convenor, and Director of Innovation) to guide the partnership through this critical period; and building organizational readiness for 2026/27 contracting reforms and capitated budget opportunities. This investment directly supports health equity goals while strengthening partnership infrastructure for long-term sustainability.
