Community Navigator and Wellbeing Service

Peterborough City Council is seeking a service provider to deliver a Community Navigator and Wellbeing service that supports eligible adults in maintaining and improving their health, wellbeing and independence. The service uses early intervention and prevention to help people and their communities build capacity for self-care and independent management, reducing the need for statutory social care services. The service is part of a wider programme called Front Door Reviews and Community Offer, which aims to improve outcomes while reducing costs by assisting with reviews and initial care planning, prioritizing meeting needs outside of formal care provision. Providers should support the Prevent, Reduce, and Delay agenda by helping to reduce unnecessary or inappropriate use of health and social care services. The contract is for an initial 2-year period with an optional 1-year extension. Note: Due to Local Government Reorganisation scheduled for 1 April 2028, the contract may be modified to reflect changes in geographical, financial and service scope, in accordance with the Procurement Act 2023. Any modifications will not take effect before April 2028.
