Supported Housing for Homeless and At-Risk Individuals in Hartlepool

Hartlepool Borough Council is seeking a provider to deliver integrated supported housing services for individuals who are homeless or at risk of homelessness with medium to high support needs. The service combines accommodation-based housing and dispersed supported housing to create a coordinated pathway of care across the Borough. The provider will source and manage all accommodation while delivering person-centred, trauma-informed support to help service users stabilise their circumstances, develop independent living skills, and progress into sustainable long-term housing. Key service components include prevention and early intervention, flexible and outcome-focused delivery, and collaborative partnership working with statutory and voluntary organisations. The successful bidder will be responsible for improving service users' health, wellbeing, and independence while directly supporting Hartlepool Borough Council's strategic objectives to reduce homelessness and enable residents to live healthier, safer, and more independent lives. This is a single contract opportunity with emphasis on strong move-on outcomes and integrated service delivery.
