Hoarding Support Service for Torfaen Residents

Torfaen Council seeks a provider to deliver a specialist hoarding support service for local residents struggling with hoarding behaviours that impact their wellbeing and independence. The service will support up to 25 people simultaneously for a maximum of 2 years or until goals are achieved, whichever is sooner. Support will be prioritised based on presenting needs including threat of homelessness, crisis situations, and medium to low needs. The service comprises two elements: (1) intensive housing-related support for individuals with hoarding difficulties affecting daily living, and (2) responsive follow-up support delivered virtually, by telephone, or in-person for those who have previously accessed the service, with mandatory monthly contact for a limited 6-month period. The provider must demonstrate specialist knowledge and experience in hoarding support, maintain excellent links with health, social care, and third-sector organisations including primary care, GPs, local community nurses, and social services. The service will collaborate with the Council's Community Team to understand hoarding prevalence in Torfaen, develop community networks, provide early intervention and education, and build community capacity and resilience. The service should align with the Council's Communities agenda, ensuring coordinated service delivery across Torfaen and avoiding duplication.
