    Brief

    NHS Herefordshire & Worcestershire Integrated Care Board is seeking a qualified provider to deliver a comprehensive Post-Diagnostic Dementia Support Service across Herefordshire and Worcestershire. This is a 3-year contract with an optional 2-year extension, commencing 1st November 2026. Service Overview: The service will support people with memory concerns, those living with dementia, and their carers through a holistic, person-centered approach. The provider will deliver integrated support across the entire dementia journey, including specialized young onset dementia services. The service will empower individuals to maintain meaningful, purposeful lives while supporting their self-management and care coordination. Service Delivery: The provider must establish a team including dementia advisors/navigators, a manager, and administrative support. All staff must be suitably experienced and qualified. A comprehensive induction programme is required for all staff at contract commencement and for new employees throughout the contract. Mobilisation: Service mobilisation will occur between 1 July and 31 October 2026, with formal contract commencement on 1 November 2026. Financial Details: Total contract value is £3,960,130.00 excluding VAT for the 3-year period (including the optional 2-year extension). VAT is not applicable. This procurement was conducted as a competitive process under The Procurement Act Regulations 2023.
