    This is a comprehensive community equipment and minor adaptations service for adults and children across four boroughs and adjacent authorized areas. Community Equipment Services: Providers must deliver the complete lifecycle of equipment services including: sourcing and procuring stock and specialized equipment; storing, distributing, delivering, installing, fitting and adjusting equipment; collecting, cleaning, decontaminating, refurbishing and recycling equipment; performing repairs, servicing and preventative maintenance; conducting technical assessments and specialist engineering visits; and supporting discharge planning, reablement, prevention, urgent community response and palliative care. Out-of-area equipment deliveries are available for individuals under commissioner responsibility, subject to approval. Minor Adaptations Services: Providers must establish a system to: accept electronic referrals from prescribers; coordinate with prescribers, clients and representatives to schedule work completion; obtain necessary property permissions before work begins; prepare Construction Phase Plans and risk assessments compliant with Principal Contractor requirements; notify prescribers of completed work; communicate any delays or difficulties in client contact; and report any problems or variations encountered during job completion.
