Assertive Outreach Service for Rough Sleepers

Caerphilly County Borough Council is seeking a service provider to deliver an outcomes-based Housing Related Support (HRS) Assertive Outreach Service for rough sleepers. The service aims to help vulnerable people, including those experiencing homelessness, mental health challenges, substance misuse, and other complex needs, to live as independently as possible. The provider must deliver a proactive, preventative approach that builds independence rather than dependency, with support tailored to individual needs and gradually reduced as clients progress. Key support areas include helping clients secure and maintain housing, maximize income, build confidence and skills, and access training and employment opportunities. The service should demonstrate efficiency and value for money while contributing to wider community wellbeing objectives aligned with Welsh Government legislation. Service providers must submit an innovative, creative solution within the Tender Document Pack framework, demonstrating how they will achieve measurable outcomes for service users.
