Key responsibilities include: assessing individual needs on hospital wards; helping arrange practical support such as cleaning, locksmith services, or food delivery; connecting people with benefits advice and emergency funding; coordinating with community services like meal programmes and befriending; preparing homes for discharge through decluttering and equipment coordination; and supporting individuals to build independence after returning home. The provider must be skilled at collaborative problem-solving, navigating complex practical issues, and working effectively with multiple partners to enable successful, sustainable discharges.
