High-Intensity User Support Services

This contract seeks a provider to deliver High-Intensity User (HIU) services that address health inequalities and reduce pressure on urgent and emergency care pathways. The service will support patients facing non-medical barriers to health, including housing instability, social isolation, loneliness, and deprivation, alongside those with poor physical and mental health. By helping these vulnerable patients access appropriate care and support, the service will free up frontline clinical resources, reduce avoidable emergency department visits and hospital admissions, and lower overall costs. Providers should demonstrate capability in holistic patient support, health equity, and integration with emergency care systems.
