    Cardiff and Vale University Health Board seeks an experienced strategic partner to support a major transformation programme shifting healthcare delivery from reactive, illness-based care to preventative, population health-driven, community-centered models. The successful partner must demonstrate expertise in community and primary care models, national and international experience, practical delivery capability, community engagement, strengths-based co-production, programme management, and strategic leadership. Over 24 months, the partner will support four key areas: (1) Programme Establishment—creating clear projects, governance structures, shared ambition, engagement plans, and benefits tracking; (2) Planning & Delivery—developing care models, workforce plans, clinical governance approaches, testing neighbourhood models, developing new leadership capabilities, introducing new ways of working, challenging traditional delivery models, creating business cases, and scaling good practice; (3) System-wide Enablers—identifying and solving requirements for estates, digital, and commissioning across the system; and (4) Bespoke Support—providing critical friend oversight, programme assurance, clinical expertise, access to national policy thinking, and connections to other transformation programmes. This is a complex, high-impact opportunity for partners with proven capability in health system transformation and integrated care delivery.
