Digital Discharge Service Workflow Management Platform

WM5G Ltd is procuring a configurable workflow management system to streamline hospital discharge-to-home pathways across NHS organisations. The solution will consolidate multiple fragmented supplier systems into a single operational platform where staff can manage referrals, patient workflows, consent, tasks, communications and performance reporting. The procurement includes software, implementation, integration, hosting, training, support and maintenance. The platform will serve as the central orchestration layer for discharge services, integrating with existing Home Readiness, Remote Monitoring and Asset Management systems while providing unified visibility of patient journeys across multiple organisations. Key capabilities include end-to-end discharge workflow management (from patient identification through referral, triage, enrolment, discharge, monitoring and completion), referral and onward-care management, task automation, consent management, advanced reporting and analytics, role-based access control, and data quality management. The solution must adopt an API-first architecture, comply with HL7 FHIR interoperability standards, and be hosted on UK cloud infrastructure with strong identity management and secure external system integration to minimise vendor lock-in. Mandatory compliance with NHS and regulatory standards including DCB0129, DCB0160, DTAC, DSPT, Cyber Essentials Plus and UK GDPR is required. The solution should deliver improved patient flow, reduced discharge delays, enhanced patient and carer experience, decreased administrative burden, improved operational oversight and scalability across multiple regions and thousands of patients. Contract award will be based on Most Advantageous Tender assessment of suppliers' ability to meet architectural requirements and operational outcomes.
