Brief
NHS Norfolk and Suffolk Integrated Care Board is procuring a provider to deliver clinical hypertension and lipids management services as part of their large-scale cardiovascular disease prevention project. The service uses a population health management approach to identify and manage patients at risk of cardiovascular events.
Key Service Requirements:
- Supply and manage clinical prescribers to initiate and optimise medicines for hypertension and lipid management
- Work collaboratively with the ICB Cardiovascular Disease team, Protect NoW team, and General Practice
- Prescribers must be qualified to initiate relevant medicines in line with Norfolk and Waveney clinical criteria and national guidance
- Conduct systematic outreach to eligible patients identified through the ICB's Eclipse population health management software
- Support personalised care approaches including pharmacological and social prescribing interventions
- Liaise with GP practices regarding ongoing patient management and monitoring
- Support patient engagement through digital and print communications to improve medication adherence and lifestyle changes
This is identified as a system priority to improve population health outcomes, extend life expectancy, and reduce pressure on the health system through early detection and management of cardiovascular risk factors.
