    Brief

The Council is commissioning targeted careers support and early intervention services for vulnerable pupils with additional needs during their transition from secondary school to post-16 education or training. The service will help reduce disengagement, improve wellbeing and life chances, and support borough objectives for children and young people. The service will work with approximately 15 pupils per secondary school, focusing on Year 10 and Year 11 students working towards GCSE grades 1-3 who have identified additional needs or receive SEND support. Key deliverables include: identifying suitable post-16 pathways (college, training, or apprenticeships); supporting pupils at risk of disengagement to remain engaged in education; assisting with college and training applications; and ensuring each young person has a clear, realistic and supported plan in place by September of Year 12. Support will run from Spring of Year 10 through to Autumn of Year 11, with early identification and planning of post-16 options at the start of Year 11.
