Community-Based NHS Health Check Outreach Service in Stoke-on-Trent

Stoke-on-Trent is seeking healthcare providers to deliver NHS Health Checks in community settings outside traditional general practice. The NHS Health Check is a preventive health assessment for adults aged 40-74 without existing cardiovascular or metabolic disease, designed to identify and manage risk factors for heart disease, stroke, kidney disease, and type 2 diabetes. You would be responsible for: - Hosting approximately 30 community events annually across Stoke-on-Trent - Delivering approximately 3,000 health checks per year through community outreach - Targeting areas with low service coverage, high health needs, or low uptake - Maintaining records and sharing data with the Local Authority and relevant GP practices Successful providers must demonstrate: - Knowledge of health and social care referral pathways - Ability to build relationships and engage with local communities - Flexible delivery approaches with varied invitation methods to maximize uptake - Capacity to support individuals with lifestyle interventions following their health check This is a hybrid payment model combining event setup costs and per-delivery payments. The contract is being tendered under the Provider Selection Regime 2023.
