    Brief

    This contract seeks a service provider to deliver comprehensive support for rough sleepers and single homeless people across the Borough of Knowsley. The service includes three core components: (1) responsive outreach services to rough sleepers and investigation of rough sleeping reports; (2) intensive housing navigation support for rough sleepers and single homeless individuals to help them secure stable housing; and (3) floating support for approximately 26 bed spaces across 11 shared accommodation properties for formerly homeless people with low to medium support needs. The service provider must help service users develop tenancy readiness and independent living skills, enabling them to secure and sustain independent housing or maintain their shared accommodation. Success is measured by positive housing outcomes and service users' ability to live independently. The provider must actively coordinate with partner agencies including Adult Social Care, Children's Social Care, Community Mental Health Teams, Drug and Alcohol Services, Housing Solutions, GPs, and other health services to ensure integrated, person-centered support.
