Support Coordinator Service for Children and Young People with Learning Disabilities or Autism

This is an opportunity to provide a Key Worker Service supporting children and young people aged 0–25 with learning disabilities and/or autism. The service will deliver coordinated, person-centred support across multiple agencies, aligned with NHS England's Keyworker Programme requirements. Key Workers will act as single points of contact, helping to navigate services, coordinate care plans, and ensure young people and their families receive consistent, joined-up support. The role requires experienced staff capable of working collaboratively with health, education, and social care partners to improve outcomes and reduce fragmentation of services.
