    Brief

    Hampshire County Council is seeking a qualified provider to deliver 300 occupational therapy (OT) assessments and 200 Care Act assessments and/or reviews. This contract aims to reduce assessment backlogs, support timely care planning decisions, and promote service user independence and wellbeing. The provider will deliver the following services: (1) Care Act assessments and support planning for a predefined number of service users; (2) Care Act reviews of support plans; (3) Mental Capacity Act assessments where required; (4) Carers Assessments where necessary; (5) support with Continuing Health Care (CHC) Decision Support Tool processes; and (6) comprehensive Occupational Therapy Assessments including initial assessments, functional assessments, moving and handling assessments (including single-handed care), and equipment and minor adaptations assessments. Services will address diverse social care needs including frailty, learning disabilities, physical disabilities, and mental health. All Care Act Assessments must be completed by 31 December 2026, and all Occupational Therapy Assessments by 31 March 2027. An optional extension period may allow the Council to commission up to 200 additional assessments at no guarantee.
