    Brief

    The Community Home First service supports the timely discharge of patients (Pathway 0 and Pathway 1) from hospitals in Cheshire West and Chester back to their homes. The service is delivered by Community Sector Coordinators (Pathfinders) embedded within hospital discharge teams, who act as liaisons between hospitals and community and voluntary sector organizations. Pathfinders triage patient needs, coordinate voluntary sector resources and volunteer support, and remove barriers to hospital discharge. Following discharge, the service provides community-based support during the critical first weeks at home to help patients recover safely and maintain independence. This opportunity is for organizations or suppliers who can provide qualified coordinators, community volunteers, and integrated discharge support services.
