    Brief

    This opportunity invites suppliers to provide an intensive supported housing programme for adults with serious mental illness requiring longer-term rehabilitation in the east of the county. The successful supplier will be responsible for: providing a building containing 10-12 self-contained residential units; delivering 24/7 staffed care and support; accommodating patients transitioning from hospital discharge; and supporting residents for up to 2 years during their rehabilitation journey. Local rehabilitation community teams will work alongside the care staff to provide coordinated, intensive support.
