Supported Housing and Tenancy Support for Adults Leaving Hospital Care

Lincolnshire Partnership NHS Foundation Trust is seeking a provider to deliver assisted accommodation, housing-related support and tenancy support services for eligible adults transitioning from inpatient and rehabilitation services. The service will help service users secure safe, affordable accommodation and maintain sustainable tenancies, reducing reliance on hospital services and supporting independent community living. The provider must manage accommodation sourcing, discharge planning, and tenancy sustainment for approximately 15 concurrent service users while tracking outcomes at 3, 6 and 12 months, managing void periods efficiently, and demonstrating value for money. The current service has achieved 93% average occupancy with 27 referrals since April 2024 and a 74% acceptance rate. This contract supports national and local priorities to shift care from hospitals to communities, reduce unnecessary delays in discharge, improve service user independence, and address housing as a key health determinant. Referrals will come from LPFT teams and partner agencies involved in discharge, housing, welfare, health and social care planning.
