The London Borough of Islington seeks an experienced support provider to deliver street outreach services for people experiencing rough sleeping. The service operates at unsocial hours (early mornings and late nights) to meet individuals where they sleep and provides rapid response support to help people access emergency and long-term accommodation. Key responsibilities include: locating rough sleepers through Street Link referrals and partner notifications; conducting welfare checks and building rapport with resistant service users; providing case management with dedicated lead workers; making accommodation referrals; conducting regular contact to prevent return to the streets; collaborating with council specialists and rough sleeping partners; participating in severe weather protocols; maintaining CHAIN database records; and submitting monthly and quarterly reports. The service directly supports the Council's Homelessness Prevention and Rough Sleeping Strategy 2024-2029 and Islington Together 2030 Plan, helping to achieve the Government's national target of halving long-term rough sleeping. The provider must demonstrate experience delivering effective street outreach in London and working with vulnerable individuals facing multiple disadvantages including poor mental or physical health, substance misuse, domestic abuse, and criminal justice involvement.
