Adult Primary Care Physiotherapy - Specialist Continence Service

This is a continuation contract with Countess of Chester Hospital NHS Foundation Trust for delivering specialist urogynaecology physiotherapy as part of an integrated Adult Primary Care Continence Service serving the Cheshire West population. The service is a critical component of the wider continence pathway operated by the West Cheshire Health Collaborative and works in close partnership with Cheshire and Wirral Partnership NHS Foundation Trust. The provider must maintain existing multidisciplinary team integration, shared clinical pathways, referral and triage procedures, clinical governance, and collaborative working arrangements. This is not a standalone physiotherapy service and cannot be separated from the integrated continence model without fragmenting care, creating process duplication, and disrupting patient management. Countess of Chester Hospital NHS Foundation Trust is uniquely positioned as the only provider currently embedded within and able to deliver the specialist physiotherapy component while maintaining the established integrated continence model. The service depends on existing clinical relationships, referral pathways, governance structures, and multidisciplinary practices developed within the West Cheshire Health Collaborative framework, which cannot be reasonably replicated by alternative providers without significant service redesign.
