Community Health Services Contract for West Lancashire

NHS Lancashire and South Cumbria Integrated Care Board (ICB) is seeking a single provider to deliver integrated community health services across West Lancashire, a predominantly rural district with a population of approximately 118,000. The contract covers adult physical health services and paediatric podiatry, including three key service specifications: Community Diabetes Service, Community Stroke and Neuro Rehabilitation Service, and Intermediate Care Therapy Service. These services support people from birth to end of life, with a focus on enabling residents to remain independent in their own homes while receiving personalised care and support for complex health and care needs. The opportunity is valued at £10,997,183 per annum, with a total contract value of £54,985,915 over a 3-year term plus potential 2-year extension. West Lancashire comprises the new town of Skelmersdale, the historic market town of Ormskirk, and numerous rural villages in the Northern Parishes.
