Medical Equipment Supply Contract for Warsaw South Hospital

This contract involves the supply of medical equipment for the Warsaw South Hospital as part of the KPO Project D1.1.1 – part XII. The contract includes the following tasks: 1. Operator chair; 2. Washer for ducks and basins; 3. Patient heater; 4. Equipment for CPR training; 5. Warmer for infusion fluids; 6. Mobile shadowless surgical lamp; 7. Wheelchairs. Detailed descriptions of the items are provided in attachment 2, 'Assortment and Price Form,' and attachments 8A-8G, which contain the technical specifications. The contract includes the sale, delivery, and installation of medical equipment in designated areas of the Warsaw South Hospital, as well as the setup and testing of the equipment before technical acceptance by authorized personnel. The contractor must ensure that the equipment is commissioned by the manufacturer's authorized service if required for warranty purposes. The contract also requires the provision of complete technical documentation, operating instructions in Polish (in both paper and/or electronic form), warranty documents, and training for the staff designated by the buyer on the operation and maintenance of the equipment, along with the provision of warranty service as per the buyer's requirements.
