The contract involves the collection and management of municipal waste from residential property owners in the Trzebownisko municipality for the period from January 1, 2025, to December 31, 2025. Suppliers must submit a complete offer, which includes: 1) A proposal prepared according to the template in Annex 1 of the SWZ - Offer Form. Other required documents include: 3) A declaration in the form of the European Single Procurement Document as per Annex 2 of the SWZ; 4) A declaration as per Annex 7 of the SWZ, if applicable; 5) A declaration confirming no grounds for exclusion as per Annex 8 of the SWZ; 6) A power of attorney or other document confirming the authority to represent the contractor(s) jointly applying for the public contract, as specified in Article 118 of the law, if applicable; 7) An information clause resulting from Article 13 of the GDPR when collecting personal data directly from the individual concerned, related to the public procurement procedure, as included in Chapter XXIX of the SWZ.
