Supply of Infusion Fluids

This contract involves the supply of infusion fluids. The order is divided into 12 tasks, as detailed in Annex 1 of the Terms of Reference, which includes the assortment and pricing form. Detailed descriptions and product parameters, including required forms/volumes, units of measure, and quantities, are specified in the assortment and pricing form. The specific conditions and rules for executing the contract are outlined in the Template Agreement, which is Annex 5 of the Terms of Reference. The series and expiration date must be indicated on the unit packaging and invoice. The expiration date of the offered products must be no shorter than 12 months from the delivery date to the Ordering Party. The contractor will deliver the products to the Hospital Pharmacy Warehouse. The offered products must be approved for circulation and use as medicinal products for each required form/volume according to the Pharmaceutical Law of September 6, 2001. The medical product related to Task 1, item 3, must be approved for circulation and use in Poland according to the Medical Devices Act.
