Community Wellbeing and Social Prescribing Service for Wrexham County Borough

This is a re-tendered opportunity to deliver a countywide social prescribing and community wellness service for residents aged 25 and over in Wrexham County Borough. The previous procurement process was terminated with no contract awarded. The service will provide personalized, non-clinical wellbeing support aligned with the National Framework for Social Prescribing. Key service elements include: conducting structured 'What Matters' conversations with individuals; developing co-produced, personalized wellness plans; delivering strength-based, therapeutic support to improve mental, physical, emotional, and social wellbeing; and providing proactive, hands-on interventions that go beyond basic signposting. Practitioners will work collaboratively with service users to understand their needs, overcome barriers, build resilience, and achieve meaningful wellbeing outcomes. The service targets individuals with identified holistic wellbeing needs and emphasizes therapeutic, community-embedded delivery.
