Community Outreach Nursing Services for Homelessness and Substance Use Support

This opportunity seeks to establish or expand a Substance Use Outreach Nurse team that delivers high-quality nursing care directly to vulnerable populations experiencing homelessness and substance use challenges. The successful provider will work collaboratively with housing services, safeguarding teams, primary care practitioners, pharmacies, and community partners to ensure integrated, person-centered care. Services should include outreach-based nursing assessments, treatment support, health education, medication management coordination, and connections to wraparound services. The team will operate in community settings rather than traditional clinical environments, meeting clients where they are to reduce barriers to care access and improve health outcomes for this underserved population.
