Primary Care Services for Mental Health and Learning Disability Inpatients

This service provides on-site or accessible primary care support to patients with severe mental illness and/or learning disabilities who are resident in designated inpatient units. The service ensures continuity of care for patients who are unable to maintain contact with their own GP, offering medical consultations, prescriptions, health monitoring, and coordination with specialist mental health and learning disability services. Providers will work within the inpatient facility to deliver timely, accessible primary healthcare and support discharge planning.
