Medical Supplies Contract for Healthcare Facility in Myszków

This contract involves the ongoing supply of disposable medical products for the Independent Public Healthcare Team in Myszków. The procurement is divided into 49 packages, with contracts for each package to be awarded for a period of 12 months after the conclusion of this procurement process. Detailed descriptions of the goods and delivery conditions are provided in the pricing form (Attachment 1.1) and the draft contract terms (Attachment 2). All requirements outlined in these documents are mandatory, and failure to meet them will result in the rejection of the offer. Suppliers must submit a completed offer form (Attachment 1) and a pricing form for the package(s) they are bidding on (Attachment 1.1). Additional documentation required includes a declaration in the form of the European Single Procurement Document (ESPD) and a statement regarding exclusion grounds based on specific EU regulations. If multiple suppliers are bidding together, each must submit the required declarations separately. The contracting authority will request additional evidence from the highest-rated supplier before awarding the contract. Offers must be submitted electronically with a qualified electronic signature.
