Medical Equipment Supply for Healthcare Facility in Łuków

This contract involves the supply of medical equipment for the Independent Public Healthcare Facility in Łuków. The detailed specifications of the required equipment are outlined in Attachment No. 2 of the specification, which includes a list of technical and functional parameters. The contract is divided into several packages: - Package 1: Real Time PCR System - Package 2: Blood and Bone Marrow Staining Device, Microscope Camera, Gas Analyzer, POCT System, Blood Centrifuge, Non-contact Vein Scanner, Blood Culture Device, Single-Arm Surgical Lamp - Package 3: Sorter with Transport System for Analyzers - Package 4: X-ray Reporting Station, License for Reporting Stations General requirements for the equipment include: 1) Must be brand new, produced no earlier than 2024, marked, and ready for operation upon installation without any additional purchases or investments. 2) Must be unused, not previously exhibited or presented, not refurbished, and free from physical and legal defects. 3) Must meet the relevant legal requirements for use in healthcare facilities. 4) Must have a medical device declaration and a CE certificate issued by a notified body, in accordance with the Medical Devices Act of April 7, 2022. 5) Must ensure the safety of patients and medical staff and provide the required level of medical services. 6) On the date of the offer submission, it should not be planned for withdrawal from production or sale by the manufacturer. The contractor is required to secure their offer with a bid bond of: - Package 1: 1200 - Package 2: 7000 - Package 3: 7000 - Package 4: 2000 The bid bond must be submitted before the deadline for offers and can be provided in one or more of the following forms: 1) Cash; 2) Bank guarantees; 3) Insurance guarantees; 4) Sureties provided by entities as specified in Article 6b, paragraph 5, point 2 of the Act of November 9, 2000, on the establishment of the Polish Agency for Enterprise Development. The required delivery time for the order is up to 56 days from the date of contract signing.
