Medical Laser Equipment Purchase for Eye Department

This contract involves the purchase of medical equipment and apparatus for the Eye Department of the Provincial Specialist Hospital No. 3 in Rybnik, Poland. The specifications for the supplies are detailed in Attachment 1.1, which includes the pricing form. Additional terms and conditions are outlined in Attachment 2, which contains the draft contract provisions for this public procurement. The contract is divided into six packages, and agreements for the awarded packages will be finalized after the conclusion of this procurement process. Detailed descriptions of the contract items are provided in the following attachments: 1) Pricing forms (separate for each package) – Attachment 1.1, 2) Technical specifications (separate for each package) – Attachments 1.2 a - 1.2 f, 3) Quality assurance and service conditions – Attachment 1.3, 4) Draft contract provisions – Attachment 2. This project is funded by the budget of the Silesian Voivodeship. The contracting authority reserves the right to cancel the procurement process if the public funds intended for financing the whole or part of the contract are not granted. According to Article 139 of the Public Procurement Law, the contracting authority will first evaluate the offers and then qualify the contractor whose offer is rated highest, checking for any grounds for exclusion and compliance with the participation conditions. In compliance with Article 24, paragraph 6 of the Whistleblower Protection Act, the contracting authority informs that a procedure for internal reporting of legal violations has been adopted at the Provincial Specialist Hospital No. 3 in Rybnik.
