    Brief

    This project involves the construction and expansion of the road network connecting Warsaw Street and St. John Street in Gliwice. The work will be carried out based on the project documentation and land development plan titled: 'Construction of Municipal Road No. 130519S, connecting Warsaw Street with St. John Street in Gliwice', for which the Mayor of Gliwice issued a ZRID decision AB-4/2022 on December 1, 2022. Detailed information about the contract can be found in attachments 7 and 8 to the SWZ. 1) The offer must be prepared clearly, using the offer form as per ATTACHMENT NO. 1 to the SWZ. The content of the offer must comply with the terms of the contract. All required documents must be attached to the offer. NOTE: Since the Contracting Authority provides its own 'Offer Form' (not through the interactive 'Offer Form' available on the e-ordering platform), a message may appear during the submission process stating: 'Do you want to continue? The procedure does not have a published form for this stage of the procedure. The file [the name of the file appears here] is not a valid interactive form generated on the Platform.' In this case, select the option 'Yes, I want to continue.' 2) Along with the offer, the following must be submitted: a) a cost estimate, b) authorization for the persons signing the offer must directly result from the registration documents. Otherwise, a relevant power of attorney must be attached to the offer, c) a commitment from the entity providing resources - as per ATTACHMENT NO. 11 to the SWZ (only if the contractor refers to the commitment of the entity providing resources), d) a statement from contractors jointly applying for the contract, as mentioned in Article 117, paragraph 4 of the Pzp Act - as per ATTACHMENT NO. 4 to the SWZ (only for entities applying jointly, e.g., consortium, civil partnership). 3) The contractor submits the offer via the 'Offers/Applications' tab, visible in the procedure preview after logging into the Contractor's account. After selecting the 'Submit Offer' button, the system presents an offer submission window allowing the transfer of electronic documents, which contains two drag&drop fields for adding files. IMPORTANT! To submit an offer, the contractor user must have the permission 'Submitting offers/applications/competition works.' 4) The contractor adds the selected and previously signed 'Offer Form' in the first field ('Completed offer form'). In the next field ('Attachments and other documents submitted in the offer by the Contractor'), the contractor adds the remaining files that constitute the offer or are submitted with the offer. 5) If documents containing business secrets are submitted with the offer, the contractor must provide them in a separate and appropriately marked file to maintain confidentiality, along with a note in the file name 'Document containing business secret.' Both the attachment containing the business secret and the justification for the business secret reservation must be added in the field 'Attachments and other documents submitted in the offer by the Contractor.'
