Medical Needles Supply and Equipment Lease

This contract involves the supply of biopsy needles along with the leasing of a system for the "Municipal Hospital in Rabka-Zdrój". The detailed specifications, including quantity and assortment, are outlined in the pricing and assortment form attached as Annex 1 to the Terms of Reference. The quantities provided in Annex 1 are estimates for the duration of the contract and will help determine the offer price. The buyer reserves the right to cancel the purchase of certain items based on actual needs, which depend on patient conditions and medical justification. The demand for specific items is closely linked to the occupancy of hospital departments and the types of patients. The buyer also reserves the right to change the quantities ordered among different items in the package. The offered goods must meet the requirements specified by the buyer in the Terms of Reference, including being new, unused, of proper quality, free from defects, and compliant with Polish law, particularly the Medical Devices Act. The goods must have a minimum shelf life of at least three-quarters of the manufacturer's nominal shelf life and must be labeled in Polish with necessary safety information. If original documentation is in another language, the supplier must provide a Polish translation. The supplier is responsible for any damages caused by non-compliance with these requirements. Deliveries will occur progressively, based on the buyer's needs, at the supplier's cost and risk, to the buyer's location. The current delivery address is ul. Słoneczna 3, 34 – 700 Rabka – Zdrój, but the buyer may change the location during the contract for hospital renovations. All delivered items must have the required legal documents, including compliance declarations and certificates. The detailed description of the order, minimum requirements, technical parameters, and delivery conditions are specified in the pricing and assortment form and the contractual provisions attached to the Terms of Reference. The buyer reserves the right to request product characteristics and documents during the evaluation of offers and throughout the contract.
