Orthopedic Implants Supply Contract

This contract involves the supply of spinal implant sets for the repositioning and stabilization of lumbar vertebrae, as detailed in Appendix 1 of the specification. The contract is valid for 24 months from the date of delivery. Additional conditions include: the supplier must provide and deposit the implants at the buyer's location (Złotej Jesieni 1, 31-826 Kraków) free of charge during the contract period. The supplier is also responsible for free replenishment and servicing of the provided instruments. If new products not previously used by the buyer are offered, the supplier must provide free training for the buyer's staff at their location (Kraków, Złotej Jesieni 1) within 30 days of signing the contract, culminating in the issuance of certificates confirming staff participation in the training. The supplier must submit a list of attendees with signatures to the hospital's registry within 5 working days after the training. Other contract conditions are specified in the draft agreement, which is Appendix 3 to the SWZ.
