    Brief

    This contract involves the supply of pharmaceutical products as detailed in Annex 2 of the IDW - Assortment and Quantity Form. The offered products must meet the requirements specified by the buyer in the Assortment and Quantity Form, be brand new, and ready for use without any additional purchases. All products must be approved for sale and use in Poland in accordance with Polish law and European Union law. Suppliers can submit offers for individual parts of the contract, which consists of 16 parts (packages). Offers can be made for one or more parts. For parts 1-13, the buyer requires the offered medicinal products to be approved for sale in Poland as per the Pharmaceutical Law of September 6, 2001. The buyer allows the offering of oral pharmaceutical equivalents in the form of tablets, dragees, or capsules with the same drug release scheme. If the requested pharmaceutical product is no longer produced or is temporarily unavailable, and there is no equivalent product, the buyer allows the submission of the last sale price. Information about product unavailability should be noted in the comments section of the Assortment and Price Form along with the relevant item. The buyer requires the delivery of products with a shelf life of no less than 12 months from the date of delivery. In exceptional cases, the delivery of products with a shorter shelf life is allowed with prior written consent from the buyer.
